Reimbursement Procedure and Request for Reimbursement Form (PDF, 3 pages)

A Request for Reimbursement can be used for the following:

• 4-H Leader and Junior / Youth Leader training education
• Approved 4-H opportunities (except for member evaluation [MES] process)

The form requires an explanation of how the training will benefit the Columbia County 4-H Program.

Approval of the request is through the Executive Committee of Leaders’ Council at a regular Leaders’ Council Meeting.  Reimbursement is typically processed after you have already attended the training and paid for it yourself.  There is no guarantee of approval.
